If you are new (or returning) to American Heritage School and would like to apply, please CLICK HERE. A non-refundable application fee is required, and you can pay via credit card or ACH direct debit. After submitting the application, we will contact you to schedule a family interview and assessment.

- Student Questionnaire
- Parent Questionnaire
- Birth Certificate
- Demographic Information
- Immunization Record
- Recent Report Card/Official Transcript
- Student Honor Code
- Parent Photo(s)
- IEP/504 Documentation (if applicable)
- Custody Agreement Documentation (if applicable)
- Recommendation form(s) (Grades 6–11)
- Math Pre-test (Grades 9–11)
